Code of Conduct and Statement of Responsibility

    To be completed by Teen

Your physical well-being during the trip is very important to us. While outdoors during the day, you are required to wear a hat and drink water throughout the day—no exceptions. These requirements help prevent dehydration. If you are not willing to wear a hat and drink water as directed, this program may not be the right fit for you. If you do apply, which we hope you will, we need your full cooperation with this requirement.

I understand and agree to Lappin Foundation’s requirement that I wear a hat when outdoors during the day and drink water throughout the day. A yes answer is required for me to participate on the trip. Check the box for yes and sign below:

    TEEN

The Code of Conduct and Statement of Responsibility is designed to ensure that 2027 Y2I and Holocaust Education Trip to Poland is an enriching, happy and safe program for every participant. Read this carefully before signing.

I will do my best to be a productive member of 2027 Y2I and Holocaust Education Trip to Poland. My actions will reflect positively on Lappin Foundation, my family and me. I will respect our Jewish culture and values by treating every member of the 2027 Y2I and Holocaust Education Trip to Poland community with respect and compassion, and in the same manner that I wish to be treated. I will advocate for myself in order to get the most out of my experience, and I will not engage in any activity that puts my own or other people’s health or safety at risk. I will remain with the organized group at all times and recognize that my health and safety depend on it. I will observe all wake up times and curfews. I will respect other people’s belongings and space and will not touch other people’s things without their permission. I will take responsibility
for my personal belongings and understand that Lappin Foundation will not be responsible for lost or damaged property.

As a member of the Y2I and Holocaust Education Trip to Poland community, I will abide by the following rules and regulations. Failure to comply with items 1-8 will result in immediate dismissal from the program:

    1. No bullying, cyberbullying, physical sexual behavior, harassment, inappropriate contact, controlling behaviors, verbal abuse, physical abuse, intimidation, or threats of violence.
    2. No leaving Y2I and Holocaust Education Trip to Poland group without permission from a counselor, including no hitchhiking and no use of public transportation.
    3. No use or possession of: drugs, tobacco, alcohol, marijuana, controlled substances, hookahs, pipes or any other drug paraphernalia.
    4. No weapons or substances used to start fires, including any object that may be used to bring harm to another person, including, but not limited to: knives, fireworks, lighters, matches and any other potential harmful items.
    5. No theft.
    6. No new body piercing or tattooing during the trip.
    7. No smoking or vaping permitted.
    8. No hazing, or any form of violent behavior whether consensual or not.
    9. No profanity or disrespectful comments, including but not limited to those regarding sexual orientation or bigotry.
    10. No possession of pornographic materials.
    11. A participant who causes any property damage will be liable to pay for the cost of those damages.
    12. Wake up times and curfews must be observed.
    13. Directives of the trip organizers must be followed.

Lappin Foundation reserves the right to dismiss any teen who endangers themselves or another individual, engages in continued behavioral problems, or violates Lappin Foundation's rules and regulations.

    • Violation of the Code of Conduct rules #1-#8 will result in immediate dismissal from the program

    • If a teen receives three strikes for violating any of the Code of Conduct rules #9-#13, it will result in immediate dismissal from the program.

If a teen is dismissed from the program, the teen's parent(s)/guardian(s) will be responsible for reimbursing Lappin Foundation the $11,000 subsidy, plus the cost of return airfare.

I have read, understand, and agree to all of the above. I understand that my failure to adhere to the above policies can result in severe disciplinary action, up to and including my dismissal from 2027 Y2I and Holocaust Education Trip to Poland. I understand that these policies are in effect throughout the entire program. I understand that should I be dismissed for disciplinary reasons:

    • My parent(s)/guardian(s) will be responsible for repaying the full $11,000 subsidy to Lappin Foundation.

    • I will be sent back to the U.S. immediately at my parent(s)/guardian(s) expense.

    • My parent(s)/guardian(s) will pay expenses for a chaperone to escort me to the United States.

    • My parent(s)/guardian(s) will not be entitled to any refunds.

Parents, your child’s physical well-being during the trip is of paramount importance to us. Wearing a hat while outdoors during the day and drinking water throughout the day - no exceptions. If they do apply, which we hope they will, your full support and cooperation is needed on this matter. Thank you.

I support Lappin Foundation’s requirement for my child to wear a hat when outdoors during the day and for my child to drink water throughout the day.  A yes answer is required for your child to participate on the trip. Check the box for yes and sign below:

including, for purposes of illustration, and without limitation, the following:

    1. The organizers of Youth to Israel Adventure and Holocaust Education Trip to Poland are not liable or responsible for injury or damage to persons or property in connection, whether directly or indirectly, with any of the following: transportation, accommodations, tour programs, or any other services when such injury or damage results from acts of God or nature, dangers incident to air and sea, fire, breakdown in machinery or equipment, acts of government or other authorities, war,
terrorism, hostilities, civil disturbances, strikes, riots, thefts, pilferage, epidemics, quarantines, custom regulations, delays, cancellations in or changes to itinerary, scheduling, or from any other causes beyond the control of the organizers.

    2. There may be times during the trip when participants will not be under direct supervision of any staff member from Lappin Foundation. The Code of Conduct and Statement of Responsibility is expected to be followed at all times.

    3. The organizers are not liable for any damages or expenses sustained by the participant as a result of any of the aforementioned causes or any other cause that is beyond the organizers’ control, and in the event it becomes necessary, advisable, or appropriate as determined by the organizers, for any reason whatsoever, to alter the itinerary and/or other arrangements, such alterations may be made without any penalty to the organizers. Any incurred damages, or expenses, shall be borne solely by the undersigned program participant and/or their legal guardian(s).

    4. The organizers reserve the right to withdraw the program should they determine that there will not be a sufficient number of participants or for any reason, the organizers determine, in their discretion, warrant withdrawal. The organizers are not responsible for any claim related to air travel.

    6. While in Israel or Poland, only limited hospital and medical insurance coverage will be available. The organizers guarantee the existence of limited hospital or medical insurance coverage. The organizers strongly advise all parents/guardians of participants to review the insurance policy, and to acquire, or maintain, medical insurance policies that are in good standing and to ensure that overseas medical costs are covered by their respective policies. In view of statutory or contractual limitations,
which may apply to personal injury or property damage/loss, the organizers further suggest that participants acquire, or maintain, additional appropriate insurance for accidents and baggage as well.

    7. Prescription drugs must be in sufficient quantities to last for the entire duration of the trip. Some medications cannot be obtained in Israel or Poland as either they are under different name or are banned for use. Medication cannot be shipped over or if sent by mail it will take a few months to clear customs. It is the sole responsibility of the participants to ensure sufficient supply of prescribed medicine.
