___ Two Letters of Recommendation – Letters of Recommendation are accepted from individuals who have known the applicant for at least six months, including teachers, coaches, counselors, or employers. Recommendations from applicant’s friends or family members will not be accepted.

___   Student in Good Standing Statement Form – To be completed by a school administrator or guidance counselor.

___ Six signed forms – Forms must be signed by both (if applicable) parent/guardians and their teen.

2. Code of Conduct and Statement of Responsibility for Parent(s)/Guardian(s)
3.  Liability Release Form
4. Cancellation and Reimbursement Policies
5.  Consent for Administration of Approved Over the Counter Medications
6.  Contract

___ One copy of teen’s Health Insurance Card, front and back

___ One clear copy of entire picture page of teen’s passport (Note: A passport is not valid for travel to Israel if it expires on or before January 9, 2027.) If teen does not a have a passport, please apply ASAP.

Due by January 25, 2026:

___ Health Form – Medical health form signed by doctor, dated within a year of the trip (July 2025). The form is the one that the doctor gives you that lists immunizations and is used for camp or sports. If your teen is due to have their doctor’s appointment after the due date, let Sharon know the date of the appointment at 978-565-4450 or email swyner@lappinfoundation.org.

Fees – 2026 Youth to Israel with March of the Living:

____ Due November 30, 2025 – Check for $500.00 (includes $200* program fee and $300* for insurance/tips) made payable to Lappin Foundation. Mail to Sharon Wyner, Lappin Foundation, 100 Cummings Center, Suite 220-G, Beverly, MA 01915.

____ Due January 25, 2026 – Check for $800 (refundable deposit) made payable to Lappin Foundation. Mail to Sharon Wyner, Lappin Foundation, 100 Cummings Center, Suite 220-G, Beverly, MA 01915.
