Teen’s Name

In consideration of Lappin Foundation accepting Applicant and Applicant’s Parent/Guardian(s) into 2026 Robert I. Lappin Youth to Israel Adventure (Israel) or (Poland and Israel) and because the Applicant is receiving a subsidy from Lappin Foundation, the undersigned Applicant and Applicant’s Parent/Guardian(s) agree as follows:

1. Attendance at all required meetings in accordance with the schedule provided. Lappin Foundation reserves the right to change dates, times, and locations of any of the meetings. Failure to attend required meetings will result in forfeiture of the refundable deposit ($500 – Israel) or ($800 – Poland and Israel), which will not be a tax-deductible donation.
2. The refundable deposit ($500 – Israel) or ($800 – Poland and Israel) will be refundable upon the successful completion of all Program Requirements. Failure to complete the pre- and post-trip requirements will result in forfeiture of the refundable deposit which will not be a tax-deductible donation
3. A personal interview of applicant and applicant’s parent/guardians(s) is required as a pre-requisite to final acceptance into the program.
4. Dismissal of teen from 2026 Y2I (Israel) or (Poland and Israel) may result in the teen’s parent/guardian(s) repaying costs incurred by the Lappin Foundation and loss of any deposits in accordance with the cancellation policy set forth in the Cancellation and Reimbursement Policies form. The deposit will be credited toward such losses.
5. The forms are incorporated herein and made part of this Contract. The forms are as follows:

• A. Application Packet, including the completed Online Registration

• B. Two Letters of Recommendation

• C. Student in Good Standing Form

• D. Code of Conduct and Statement of Responsibility for Teen

• E. Code of Conduct and Statement of Responsibility for Parent/Guardian(s)

• F. Liability Release Form

• G. Cancellation and Reimbursement Policies

• H. Consent for Administration of Approved Over the Counter Medications

• I. Contract

6. TAX NOTICE: The receipt of the subsidy and whether or not it is income to your child should be checked with whoever prepares your tax return. Lappin Foundation will not be sending your child a Form 1099.

All statements in regard to this application and other contract documents are true to the best of our knowledge. False, misleading information or misrepresentation of the information will result in forfeiture of the subsidy ($8,000 – Israel only or $11,000 – Poland and Israel), repayments of any amounts spent by Lappin Foundation, loss of the refundable deposit and fees and dismissal from the program.

We have received, read and fully agree to all conditions and requirements as listed in the contract.
